Order Forms


There are several ways for you to order or contact us:

1. Voice: Call us at the toll free voice number below for any of your needs or questions. You can check Name Availability, then place your order with the same call.

2. Fax: Select the appropriate Order Form below, fill it in, print it from your Web Browser and Fax to our toll free fax number below. Use this method if you are paying by credit card.

3. U.S. Mail: Select the appropriate Order Form below, fill it in, print it from your Web Browser, and mail to our address below. Use this method if you are paying by check.

4. E-mail: Select the appropriate Order Form below and press the "Copy Blank Form to Mail Window" button. This will launch your mail window and place a blank copy of this form into it. Fill it in and send mail to us. Should the form not appear in mail window, you may need to take an option to have it appear. (For Example use File...Include Original Text - when using Netscape 3.0)

Warning!! - You should not send credit card information via Internet Mail. If you wish to pay by credit card, indicate such and we will contact you for your account information.
